Chronic Disease is a health condition or illness that lasts for a year or more, typically requires ongoing medical attention, and may limit daily activities. Chronic disease and conditions, such as heart disease, cancer, stroke, type 2 diabetes, obesity, and arthritis are among the most common, costly, and preventable of all health problems. About half of all adults in the United States have one or more chronic health conditions and seven out of ten deaths each year are caused by chronic disease.

Cecil County Health Department offers the following Chronic Disease Prevention Programs & Workshops:

Diabetes Prevention Program

The Health Department’s Diabetes Prevention Program (DPP) can help you lose weight, become more physically active and reduce stress.  The program is CDC-approved and has been proven effective in preventing or delaying type 2 diabetes. DPP is currently offered in-person and virtually, as a year long program, meeting weekly to begin, and monthly by the end. Living Well Self-Management Programs

The Living Well self-management programs help you learn how to live better by managing your disease. These free of cost programs meet once a week for six-weeks and are offered to adults 18 years of age and older who have or care for someone, living with a chronic condition, a cancer diagnosis, diabetes, chronic pain or hypertension. Programs are *evidence-based and available in-person, virtually, or by phone.
